Survivor Information Booklet
What is this Booklet and what do I do with it?
This ‘Survivor Information Booklet’ is a gift to you and your spouse from Sacred Heart. This booklet will contain all of your ‘end of life’ important information that your loved ones will need when you die. As an example the booklet includes where the key to your safety deposit box is hidden. Without it no one can retrieve it. It contains a list of Emergency contacts, Living Will, Power of Attorney and Insurance information.
It is intended to be downloaded to your computer where you can fill out all of the appropriate fields, print the form out and either kept in a safe place where your nearest of relatives or others can find it upon your passing.
Update the information at least once a year. Things change, this should also change.
HOW TO USE THIS BOOKLET
This booklet is designed to help you organize the important information your loved ones will need. Please fill in each section as completely as possible.
Store this booklet in a secure but accessible location. Make sure at least one trusted person knows where to find it.
Review and update this booklet annually or whenever your circumstances change.
You do not need to complete every section — fill in only what applies to you.
What do I do with the book?
First, download a copy of the book. The book is in a Microsoft Word format. You can type in each answer. If your answer is longer than the area allotted on the form - just keep typing - it will expand. If you have filled out all of the allotted source fields copy the last source field and paste it on to the last source field to create additional fields.
If you are in a section of the book that doesn’t apply to you, simply delete it.
